Upload an attachment bound to an entity POST

multipart/form-data with one `file` part, `entity_type` (project|organization|employee|contact), `entity_id`, and an optional `category`. Encrypted at rest. Honors `Idempotency-Key`: a retry with the same key and the same file replays the original response; the same key with a different file returns 422. Max 25 MB.
